A 5-year-old asymptomatic boy had apparent optic nerve head elevation on routine ophthalmology examination. Magnetic resonance imaging of the brain revealed a previously undiagnosed Chiari I malformation with crowding of the foramen magnum and low-lying cerebellar tonsils.
